PARTNERSHIP IN CARE Research shows that the best place for patients to heal or improve in their care is at home. That’s why our hospital works to plan for each patient’s discharge from their first day of admission, ensuring that an extensive care plan is in place. There are many community resources available to support treatment at home and also great facilities for patients who cannot go home but no longer need to stay in hospital. This year, JBH focused on building and expanding collaborative relationships with important healthcare providers within our Burlington community. These include the Community Care Access Centre (CCAC), Family Physicians, retirement homes and long-term care facilities, to ensure patients continue to receive the right care, at the right place, at the right time. JBH worked together with our partners to improve our understanding of the range and types of services available to each patient. Caring for our patients in hospital and then in the community, requires a commitment from all partners and care providers to continually be flexible, innovative and open to deliver the level of care our patients and families deserve. IMPROVING EMERGENCY DEPARTMENT WAIT TIMES There are thousands of people cared for in our Emergency Department (ED) every year and JBH knows how important it is to move patients to the best place in our hospital to receive their care, as soon as they can. This movement of patients has become a top priority in the hospital, as previously, patients were waiting too many hours for an inpatient bed to become available. This year, the hospital examined how we care for our patients in the ED, looking for improvements in wait times and getting patients to the right bed faster. Staff and physicians worked together to create a new model of care that improved our processes, made our work more efficient and decreased wait times for patients. With implementing the first phases of the model, JBH has already seen a 39% reduction in wait times to move ED patients into an inpatient bed, from 54.6 hours in 2014/15 to 33.1 hours in 2015/16. Soon we will be meeting the provincial level of 28.0 hours. The improvements our hospital has made have been recognized by the Province – in January, JBH was recognized as the most improved Medium-Volume Community Hospital Group. 39% reduction in wait times STATE-OF-THE-ART DIAGNOSTIC EQUIPMENT This year, our Diagnostic Imaging department benefited from enhanced technology and new equipment with the installation of two state-of-the-art CT Scanners. The arrival of these two machines is as a result of the generous $60 million commitment made by the City of Burlington to JBH’s redevelopment project. CT scans are used to visualize almost all parts of the body, as well as to plan medical treatment. What makes these CT scanners so remarkable is the digital technique and enhanced resolution. These advancements permit less invasive screening for diseases and injuries, while completing patient scans with speed and accuracy. With more than one machine onsite, JBH can better serve our patients. Out-patient and emergency patients requiring CT scans can be imaged in their respective areas, reducing patient transfers and patient wait times. This means we are providing patients with the right care, at the right time. This is just one of many examples of how JBH is enhancing the way we practice medical care. Annual Report 2015-2016 7
