The app synthesizes clinical trial data for antithrombotic therapy options and uses this data in combination with individual patient characteristics to calculate individualized annual risk of ischemic stroke and thromboembolism with a concurrent annual risk of major bleed. The app is currently available for use on iPad, iPhone, Android, and tablet. A 44-year-old man presents with a 3-month history of worsening dyspnea on exertion and orthopnea. His body mass index is 36 kg/m2 and he has obstructive sleep apnea, for which he uses a continuous positive airway pressure (CPAP) machine during sleep. Physical exam reveals a blood pressure of 102/78 mm Hg, an irregular rhythm with a heart rate ranging from 105 to 143 bpm, a grade 4/6 systolic murmur, and bibasilar rales. What would you do?
