Dear Parent/Guardian:
Each year Teaneck High School publishes the HiWay yearbook. The HiWay yearbook may include
the name, grade level, and photographs of students in Teaneck High School. In addition, the HiWay
yearbook may use photographs of students in flyers and mailings promoting the sale of yearbooks
as well as the sale of advertising space. However, parents/guardians have the right to instruct the
school in writing that this information is not to be included in the yearbook or any yearbook flyers
and mailings.
If you do not consent to the use of information pertaining to or photographs of your child 1) in the
HiWay yearbook or 2) HiWay yearbook flyers and mailings, please check the appropriate box or
boxes below. To be certain your wishes are respected, return this form to the Main Office by
September 30th. After that date the HiWay yearbook begins production and will not be able to
remove any photographs or information that has already gone for printing.
