Assess client’s knowledge and learning needs 3. Prepare the client for intervention / procedure 4. Prepare the environment 5. Gather and check the equipment 6. Decontaminate hands INTERVENTION / PROCEDURE 7. Introduce self 8. Identify correct patient/client 9. Explain procedure to client/parent/caregiver 10. Obtain informed consent 11. Protect/maintain health & safety of self and patient/client 12. Adhere to infection control requirements Diabetic Foot Assessment - Circulation & Perfusion Sensation / Peripheral Neuropathy Skin Features & Integrity Decontaminate hands & equipment if necessary Explain results / findings to the client RECORDING, REPORTING & DOCUMENTATION 13.
